This isn't quite it, but the idea is there. I love the traditional oriental-style back tattoos, particularly the koi-descending-from-upper-back ones. So... in some ways, I'm tempted to get a tattoo as a reward for weight loss (when it happens!), probably one on my back where it's not visible to everyday people, but I know it's there :)

Pain for pleasure or pleasure for pain, I guess you could say, but sometimes you just want to get some kind of permanant mark for something that's been achieved. Like some kind of Student of the Week award, but not.
